High intensity focused ultrasound (HIFU) has recently been introduced as a new treatment for treating patients who are looking to lose weight and/or have their skin looking younger. It is similar to RF in that it relies on heating tissues in order to cause apoptosis; it does not rely on heating with electricity. High-intensity focused ultrasound can deliver energy to deep layers of the skin and to fibroblasts, causing a high level of tissue coagulation without damaging the epidermis. Focused energy causes high temperatures to be generated (>65°C) within 1 to 3 seconds, leading to collagen formation in the tissue. In addition to producing a high temperature, HIFU produces a mechanical effect that causes damaged cell membranes within seconds. This promotes cell death and necrosis. The effects of these devices can be visualized, as there is always a clear line between the tissue being treated and the one that is untreated. Microcoagulation, as the name suggests, causes a skin layer to become tighter gradually by contracting and remodeling collagen.

High-intensity focused ultrasound was developed for eyebrow lifting in the early 1990s and has since been used safely to treat skin conditions in a wide variety of skin types and to improve the appearance of the abdomen and thighs. This technique is most suitable for patients who have mild to moderate skin laxity or for those who are recovering well from a wound.33 The patient should have normal skin healing and be young and healthy. Those who are very old or who have severe skin laxity are not good candidates for HIFU.
